Patient Centered Data Home (PCDH)
The WYFI team is pleased to announce that the Patient Centered Data Home (PCDH) is connected to WYFI. WYFI will be connected to the Western side of the gateway. The PCDH is a secure health data exchange initiative of CIVITAS:
- It is a cost-effective and scalable method of exchanging patient data among health information exchanges (HIEs).
- The PCDH can help patients by proactively alerting their healthcare providers when they have a health event away from home so that their health records can be available wherever and whenever they need them.
REMINDER WYFI TRAINING
Just to let you know the WYFI team will be conducting administrative training on the new Patient Comprehend system (KPI Ninja) platform which is replacing the Community Health Record (CHR).
The WYFI team will be sending out Google Meet User Training invitations to users. This training is meant to provide a demonstration of how the User Administrators are to enroll new users within the KPI Ninja Platform. The Training will be offered multiple times so the admin users have the option to select a training session that will work best for them. If there is not a good option for you, please reach out to the WYFI team at wdh-wyfi-hie.wyo.gov so that we can set up a time that best fits your schedule.
The current Community Health Record (CHR) site will be disabled on the go-live date and will redirect users to the new Patient Comprehend (PC) site. Username and password information will be sent out via email from the Patient Comprehend portal the day before the system upgrade takes place. WYFI users should remove the old URL and replace it with the new URL once we have disabled the CHR site.
