When using the Wyoming Frontier Information (WYFI) Health Information Exchange (HIE) for case management, the goals are to: 

  • Provide Event Notifications and Point of Care access to Long-Term Services and Supports (LTSS) Specialists to actively participate in discharge planning and improve care coordination for program-eligible consumers to:
    • Allow specialist to actively participate in the discharge process to determine consumer needs and avoid potential admissions to nursing facilities
    • Improve care coordination for consumers and improves their ability to return home

Contributor Role
  • An assigned case manager receives notification when her patient is discharged from a WYFI-connected hospital after a scheduled procedure, triggering her to resume case management activities
  • Case manager has access to labs, radiology and transcription from within her case management system
  • Case manager notifies home care providers to resume services using alerts within her system and including details and updates on the patient’s post-op status and needs
View-Only Role
  • A case manager receives notification when patient is discharged from a WYFI-connected hospital after a scheduled procedure
  • Case manager resumes case management activities and transcribes notes from the WYFI HIE into her case management system
