Contributor Role
- Upon arrival in the office that morning, PCP receives notification of overnight inpatient admittance to the ED at regional health facility
- PCP sets task in the system for office to schedule follow-up appointment for aftercare upon discharge to manage care and prevent re-admission
- Patient admittance, labs and radiology are all available in PCP’s electronic health record (EHR) automatically for PCP to review prior to the scheduled aftercare visit
View-Only Role
- PCP receives notification of overnight inpatient admittance to ED at regional health facility
- PCP sets task in EHR for scheduling office to set up a follow-up appointment for aftercare upon discharge to manage care and prevent re-admission
- PCP’s nurse accesses the WYFI HIE to pull down the ER information and input to the office’s EHR prior to the patient’s scheduled aftercare visit
